资格要求：1、投标人具有合法有效的企业法人营业执照、税务登记证及组织机构代码证或证照合一的营业执照，投标时需提供原件扫描件（原件备查）。投标人为事业单位的，应具有合法有效的事业单位法人证书，投标时需提供原件扫描件（原件备查）。投标人为分公司的，应具有合法有效的营业执照和上级法人单位授权书，分公司与上级法人单位只可一家参与投标，同时参与投标的，投标均无效。 如果投标人为境外注册公司，需提供有效的公司登记注册证明。2、投标人应为本次投标产品的制造商。本次招标不接受代理商、贸易商投标。本次接受所投货物的制造商全资的销售公司或控股的销售公司，以及接受所投货物制造商的母公司参与投标，此类公司视同为制造商，投标时需提供相应的证明文件。同一制造商仅允许一家投标人参与投标，否则相关的所有投标将被否决。3、如出现不同投标人的负责人为同一人或存在控股、管理关系的情况，相关投标均无效，招标人随时有权做出拒绝投标、取消投标资格、取消授标、不签订合同或在合同签订后终止合同等决定，相关投标人须无条件接受，且由此做出的任何损失由对应投标人承担。4、投标人所投产品的制造商投标时需提供合格有效的GB/T19001（ISO9001）质量体系认证证书（应包含生产或制造），并可在中国国家认证认可监督管理委员会网站(http://www.cnca.gov.cn/)核实。如果有国家相关部门发布的最新体系标准，以最新体系标准为准。投标时需提供原件扫描件（原件备查）。注：如所投货物的制造商提供的质量体系认证证书是外国第三方认证机构颁发的，则网站查询不适用。Qualification Requirements:1. The business license requirements are proposed to be revised as follows: the bidder has a legal and valid business license of enterprise legal person, tax registration certificate and organization code certificate or a business license combined with the certificate, and the scanned copy of the original shall be provided during bidding (the original copy for reference). If the bidder is a public institution, it shall have a legal and valid certificate of legal person of the public institution, and the scanned copy of the original shall be provided during bidding (for reference of the original). If the bidder is a branch company, it shall have the legal and valid business license and the letter of authorization of the superior legal entity. Only one of the branch company and the superior legal entity may participate in the bidding. If the bidder participates in the bidding at the same time, the bidding shall be invalid.If the bidder is an overseas registered company, a valid certificate of company registration shall be provided.2. The bidder shall be the manufacturer of the bid product. This tender does not accept agents, traders bid. The wholly-owned or majority-owned sales company of the manufacturer of the goods accepted this time, and the parent company of the manufacturer of the goods accepted to participate in the bidding, such companies shall be regarded as manufacturers, and the corresponding supporting documents shall be provided when bidding. Only one bidder from the same manufacturer is allowed to participate in the bid, otherwise all relevant bids will be rejected.3. If the person in charge of different bidders is the same person or there is a holding or management relationship, the relevant bid shall be invalid, and the tenderer has the right to make decisions at any time such as rejecting the bid, disqualifying the bid, canceling the award of the bid, not signing the contract or terminate the contract after the signing of the contract, etc. The relevant bidder shall unconditionally accept the decision, and any losses arising therefrom shall be borne by the corresponding bidder.4. The manufacturer of the bidder's bid products shall also have valid GB/T19001 (ISO9001) quality system certification (including production or manufacturing), which can be verified on the website of Certification and Accreditation Administration of China (http://www.cnca.gov.cn/). If there are the latest system standards issued by relevant state departments, the latest system standards shall prevail. Note: If the quality system certification certificate provided by the manufacturer of the goods is issued by a foreign third party certification body, the website query is not applicable.业绩要求：（开标时需进行信息公开）2018年1月1日至投标截止日（以合同签署时间为准），投标人所投产品的制造商应具有至少1个合同的1500磅插入式过球指示器产品的供货业绩（测量介质不限）。投标人须按规定格式提交业绩表，并提交相关业绩证明文件。业绩证明文件包括但不限于：1）销售合同复印件和2）到货验收材料。投标人所提交的业绩证明文件必须至少体现以下内容：合同首页、合同签署时间、合同签署页（应有双方签署或盖章）、制造商名称、货物名称（磅级1500磅、形式为插入式过球指示器）及到货验收材料，到货验收材料至少包含：对应的增值税发票复印件（如有清单须提供。发票须体现发票号、物资名称、买方卖方信息；发票应包含合同号或订单号，或发票金额与合同或订单金额一致）或买方接收证明或验收证明等，买方接收证明或验收证明应有甲方签字或盖章。若合同为年度/项目协议，除提供协议文本外，还应至少提供该协议对应的至少1个已完成订单的订单页。一个订单视为1个业绩。Performance requirements(Performance track record will be released to all bidders during bid opening): From January 1, 2018 to the bid deadline date (subject to the contract signing date), the manufacturer of the bidder's bid products shall have at least 1 contract 1500 pound plug-in ball indicator to measure the delivery performance of the product with no limitation on the medium, and meet the technical requirements. The bidder shall submit the performance table in the prescribed format, and submit the relevant performance certification documents. Performance proof documents include but are not limited to: 1) copies of sales contract and 2) arrival acceptance materials. The performance certification documents submitted by the bidder must at least reflect the following contents: Contract first page, contract signing time, contract signing page (signed or sealed by both parties), manufacturer name, goods name (pound class 1500 pounds, in the form of a plug-in ball indicator), technical requirements and arrival acceptance materials
